1. Print out order form and price sheet
2. Pick a style you like the best.
Check the number of shirts it will take for the size you need. A photo counts as one shirt.
3. Add optional items to make it more personal. Stripes in the border, extra quilting (Style C only), photos, Love
Patches, sleeve if it is to be a wall hanging with a rod. (I staple my quilts to the wall so they do not need a sleeve)
4. If you have a question or want to let me know the order is on the way, you can use the box below.
Tell me what your main color will be, so I will for sure have enough fabric on hand when your shirts arrive.
5. Pin notes to the shirts if you want them placed in the center or in a certain order. Draw a diagram on
the back of the order form. If there is no preference, I usually balance the colors through out the quilt.
6. Enclose a check with a deposit (half) or the full amount (Make check to Bonnie Mulvany). OR Send
your credit card (Master Card / Visa) and I will charge it to your card when I am ready to ship it to you OR
you can pay through PayPal with any major credit card. PayPal is a secure way to pay online.
You do not need a PayPal account and there is no charge to you. The quilt needs to be paid in full before I can ship it.
Just let me know when you put in the order how you want to cover any balance remaning.
7. Ship the shirts along with the order form to Bonnie Mulvany, 2701 Reed St., Wheat Ridge, CO 80033
I will let you know when they arrive and if I have any questions. I'll give you a projected return date at that
time.
